Intervention
Home blood pressure telemonitors with data transmitted to pharmacists who adjusted antihypertensive therapy accordingly, provided for 12 months.

Control of systolic BP to less than 140 mm Hg and diastolic BP to less than 90 mm Hg (<130/80 mm Hg in patients with diabetes or chronic kidney disease) at both 6 and 12 months.surrogate
