Key result
A tailored left ventricular-only pacing strategy facilitated by Bachmann bundle atrial pacing improved ejection fraction from 30% to 45-50% and NYHA class from III to II in a patient with transient LBBB.
Why the study?
Patients with systolic CHF and transient LBBB represent a difficult to treat and understudied population with unique challenges in CRT programming.
Case Report (n=1)
Bachmann Bundle atrial pacing can shorten the PR interval to facilitate LV-only pacing algorithms in CRT patients with transient LBBB, leading to improved clinical and echocardiographic outcomes.

May support individualized CRT in transient LBBB; hypothesis-generating and requires prospective validation.
Cardiac resynchronization (CRT) therapy is an established treatment for systolic congestive heart failure (CHF) with left bundle branch block (LBBB) QRS morphology. When observed on serial EKG tracings over time, LBBB is presumed to be permanent, however in some patients such patterns are transient in nature. Biventricular pacing treats the dyssynchrony caused by LBBB, but does not provide better electrical synchrony relative to native activation of the left ventricle through the His-Purkinje system with a narrow QRS.

Manogue et al. (2026) conducted a case report in Systolic congestive heart failure with transient left bundle branch block (n=1). Left ventricular-only pacing CRT with Bachmann Bundle atrial pacing was evaluated on Ejection fraction and NYHA functional class. A tailored left ventricular-only pacing strategy facilitated by Bachmann bundle atrial pacing improved ejection fraction from 30% to 45-50% and NYHA class from III to II in a patient with transient LBBB.
