ICE detects unexpectedly large LA thrombi following surgical LAA ligation.
Case Report (n=1)
This case highlights the risk of large thrombus formation in the left atrium following surgical LAA ligation and the utility of intracardiac echocardiography for detection.
May signal LA thrombus risk despite surgical LAA ligation; leaves open need for prospective surveillance studies.
The incidence of atrial fibrillation (AF) is gradually increasing because of the aging society.1 Various methods for preventing cerebral embolisms, including anticoagulants, percutaneous left atrial appendage (LAA) closure, surgical LAA ligation, and surgical LAA resection, have been widely performed.2 Surgical LAA exclusion is an excellent treatment that effectively reduces the incidence of strokes. Therefore, it is increasingly performed during mitral valve surgery and surgical pulmonary vein isolation.

Narita et al. (2025) conducted a case report in Atrial fibrillation (n=1). Surgical left atrial appendage ligation was evaluated. Intracardiac echocardiography detected unexpectedly large thrombi in the left atrium in a patient following surgical left atrial appendage ligation.
