To compare the prognostic performance of the 2001 ATPIII and the 2005 IDF definitions of the metabolic syndrome for predicting cardiovascular events in patients with coronary disease.
Conducted a prospective study of 750 coronary patients who underwent coronary angiography.
Tracked clinical vascular events over a follow-up period of 4 years.
Classified metabolic syndrome status using both the Adult Treatment Panel III (ATPIII) and the International Diabetes Federation (IDF) criteria.
Metabolic syndrome was present in 37.3% (n = 280) of patients according to ATPIII criteria and in 45.5% (n = 341) according to IDF criteria.
ATPIII criteria significantly predicted vascular events (adjusted HR 1.745 [95% CI 1.255–2.427]; P = 0.001), whereas IDF criteria did not (adjusted HR 1.189 [95% CI 0.859–1.646]; P = 0.297).
Event-free survival was significantly lower for patients meeting ATPIII but not IDF criteria compared to those meeting IDF but not ATPIII criteria (P = 0.012), with ATPIII risk losing significance after adjusting for high triglycerides and low HDL cholesterol.
