Does lidocaine hydrochloride successfully manage ventricular arrhythmias in a case of pilsicainide intoxication?
Lidocaine hydrochloride may be successfully used to manage ventricular arrhythmias resulting from pilsicainide intoxication.
Lidocaine may offer an option for ventricular arrhythmias in pilsicainide intoxication; single case leaves efficacy open to prospective study.
According to the Vaughan-Williams classification, pilsicainide is a class Ic antiarrhythmic agent that exerts its pharmacodynamic effects via selective blockade of sodium channels; furthermore, it is mostly available in East Asia.1 This drug is absorbed from the gastrointestinal tract, and 75%–86% of it is excreted from the kidney.1 Pilsicainide is considered relatively safe and effective for the treatment of supraventricular and ventricular tachyarrhythmias without organic heart disease and is commonly used to diagnose Brugada syndrome (BrS) by inducing a coved-type electrocardiogram (ECG).

Kawada et al. (2023) conducted a case report in Pilsicainide intoxication with ventricular arrhythmias (n=1). Lidocaine hydrochloride was evaluated. Lidocaine hydrochloride was successfully used to manage ventricular arrhythmias in a patient with pilsicainide intoxication.
