His bundle pacing provides a promising RVP alternative for bradyarrhythmias with >40% pacing and reduced LVEF.
Why the study?
His bundle pacing is a promising alternative to classic right ventricular pacing and cardiac resynchronization therapy for bradycardia and heart blockades, but faces limitations including a lack of large randomized trial results.
Does His bundle pacing improve outcomes compared to classic right ventricular pacing or cardiac resynchronization therapy in patients with bradycardia and heart blockades?
Does His bundle pacing improve outcomes compared to classic right ventricular pacing or cardiac resynchronization therapy in patients with bradycardia and heart blockades?
His bundle pacing represents a promising physiological alternative to traditional right ventricular pacing, particularly for patients with heart failure and a high expected burden of ventricular pacing.

Should not yet change pacing practice; leaves open His bundle pacing benefits pending randomized confirmation.
The article presents review of the literature, clinical studies and current guidelines for the usage of His bundle pacing – a new method of treatment of bradycardia and heart blockades. Despite certain temporary limitations – lack of results of the large randomized trials, necessity of the special surgeon's experience in implantation of the electrode, there are reasons to consider His bundle pacing as a promising alternative to the existing methods of treatment of bradyarrhythmias (e.g. classic right ventricular pacing, cardiac resynchronization therapy), particularly in patients with a predicted high (> 40 %) percentage of right ventricular pacing and decreased left ventricular systolic function.

Skybchyk et al. (2020) conducted a review in Bradycardia, atrioventricular block, heart failure. His bundle pacing vs. Right ventricular pacing, biventricular pacing was evaluated. His bundle pacing provides a promising physiological alternative to traditional right ventricular pacing for treating bradyarrhythmias, particularly in patients with >40% expected pacing and reduced ejection fraction.
