Recent HF hospitalization may identify high-risk HFpEF patients; hypothesis-generating for time-dependent enrichment strategies independent of EF.

Does the time interval from previous heart failure hospitalization influence the risk of cardiovascular death or HF hospitalization in patients with reduced and preserved ejection fraction?
Recent heart failure hospitalization identifies a high-risk population for future clinical trials, as the risk of cardiovascular death or readmission declines as time from prior hospitalization increases, independent of ejection fraction.
Bello et al. (2014) conducted an RCT in Heart failure with reduced and preserved ejection fraction (n=7,599). Time interval between previous HF hospitalization and randomization vs. No previous HF hospitalization was evaluated on cardiovascular death or unplanned admission to hospital for the management of worsening HF. Recent heart failure hospitalization (≤6 months) in preserved EF patients conferred a risk of cardiovascular events comparable to reduced EF patients without prior hospitalization.
