Why the study?
Identifying patients with advanced HF at high risk of death facilitates timely palliative care referrals, but models specific to end-stage HF are limited.
What are the risk factors associated with up to three-year all-cause mortality in patients with advanced heart failure?
What are the risk factors associated with up to three-year all-cause mortality in patients with advanced heart failure?
This scoping review aims to identify risk factors and prognostic models for up to three-year all-cause mortality in patients with advanced heart failure to facilitate timely palliative care referrals.
Supports risk stratification for palliative referral in advanced HF; leaves open validation of identified models in prospective studies.
Introduction: Identifying the evolving needs of patients with advanced heart failure (AdHF) and triaging those at high risk of death can facilitate timely referrals to palliative care and advance patient-centered individualized care. There are limited models specific for patients with end-stage HF. We aim to identify risk factors associated with up to three-year all-cause mortality (ACM) and describe prognostic models developed or validated in AdHF populations.
