Key result
His bundle pacing is a physiologic alternative to right ventricular pacing, though limited by concerns of threshold increases, unpredictable His capture, and excessive battery drain in young patients.
Why the study?
Evidence for replacing conventional RVP with HBP remains limited to small trials, and threshold increases causing unpredictable His capture or excessive battery drain are concerns in young, pacemaker-dependent patients.
Does His bundle pacing provide a safe and effective alternative to right ventricular pacing without excessive threshold increases or battery drain?

Does His bundle pacing provide a safe and effective alternative to right ventricular pacing without excessive threshold increases or battery drain?
While His bundle pacing is a physiologic alternative to right ventricular pacing, concerns remain regarding threshold increases and battery drain, particularly in young pacemaker-dependent patients.
His bundle pacing (HBP) has emerged in the last decade as an attractive alternative to conventional right ventricular pacing (RVP), being the most physiologic form of cardiac stimulation by the avoidance of pacing-induced intraventricular and interventricular dyssynchrony.1 Although the premises to replace customary RVP are sound, evidence is still limited, with only small, randomized trials available. Furthermore, HBP threshold increase leading to unpredictable His capture at follow-up or to excessive battery drain are a concern in young patients, especially when pacemaker dependent.

Lazzeri et al. (2024) conducted a case report in Pacemaker dependency. His bundle pacing (HBP) vs. Right ventricular pacing (RVP) was evaluated. His bundle pacing is a physiologic alternative to right ventricular pacing, though limited by concerns of threshold increases, unpredictable His capture, and excessive battery drain in young patients.
