Does radiofrequency ablation of the AV node and pacemaker implantation improve cerebral perfusion and cognitive function in patients with persistent atrial fibrillation?
AV node ablation and pacemaker implantation in persistent AF improves cerebral perfusion and neurocognitive function, likely driven by increased cardiac output.

May support AV node ablation plus pacing for cognitive gains in persistent AF; leaves open confirmation in randomized trials.
The objective of this study was to evaluate cerebral perfusion according to SPECT with 99m Tc- HMPAO in the patients with the implanted pacemaker because of persistent AF. The study included 17 patients with persistent AF who underwent brain SPECT, assessment of neurological status, cognitive function, and echocardiogram. It was done at baseline and 3 months after the establishment of a full cross-blockade by radiofrequency ablation of the atrioventricular connections and pacemaker implantation. The results showed that 99m Tc-HMPAO is useful radiopharmaceutical for assessment of cerebral perfusion in patients with persistent AF. The received data indicate that the pacemaker implantation surgery 3 months later, increasing left ventricular ejection fraction, systolic and cardiac outputs, has a positive effect on cerebral perfusion and leads to the improved neurocognitive function in patients.

Efimova et al. (2015) conducted an observational in Persistent Atrial Fibrillation (n=17). Radiofrequency ablation of AV connections and pacemaker implantation vs. Baseline was evaluated on Cerebral perfusion and neurocognitive function. Radiofrequency ablation of AV connections and pacemaker implantation improved cerebral perfusion and neurocognitive function at 3 months in patients with persistent atrial fibrillation.
