Implication

VTE after orthopedic surgery was associated with higher charges; leaves open whether prevention strategies reduce utilization.

Structured PICO

Does clinical VTE increase healthcare utilization and billed charges in patients undergoing major orthopedic surgery?

P
Population
11,960 patients who underwent total hip replacement (n=3,171), major knee surgery (n=3,955), or hip fracture repair (n=4,834) from January 1993 to December 1998.
I
Intervention
Clinical VTE (diagnosis of deep vein thrombosis or pulmonary embolism within 90 days of surgery and ≥1 prescription for warfarin or unfractionated heparin within 30 days of initial VTE diagnosis)
C
Comparator
Matched controls (matched by age and procedure type) who did not have any claims for clinical VTE
O
Outcome
Healthcare utilization (length of stay) and billed charges over a 90-day period following admission

Venous thromboembolism following major orthopedic surgery is associated with substantially increased hospital length of stay and healthcare costs, regardless of whether it occurs in-hospital or post-discharge.
