Does His bundle pacing restore native intrinsic conduction in patients with chronic conduction block?
His bundle pacing may not only normalize ventricular activation but also restore native intrinsic conduction in patients with chronic conduction block after years of dormancy.
Should not yet change practice in chronic block; leaves open need for prospective trials to confirm benefits.
His bundle pacing (HBP) has emerged as a physiological form of ventricular pacing that has been shown to be safe and feasible in clinical practice. Since it induces ventricular contraction by exciting the intrinsic conduction system, it has the benefit of reducing or eliminating both interventricular and intraventricular dyssynchrony. In patients with infranodal block1 or left bundle branch block (LBBB),2 HBP can result in normalization of QRS complex, ventricular activation, and hemodynamic parameters including left ventricular (LV) systolic function.

Ezzeddine et al. (2021) conducted a case report in Chronic conduction block and heart failure (n=2). His bundle pacing was evaluated on Restoration of native intrinsic conduction. His bundle pacing restored native intrinsic conduction and normalized left ventricular systolic function in two patients with years of chronic conduction block.
