Key result
Impaired cardiac baroreflex effectiveness during provoked blood pressure fluctuations was associated with poorer functional capacity in nonhypoxemic PAH, and was not mediated by the peripheral chemoreflex.
Why the study?
It was unknown whether the peripheral chemoreflex plays a role in cardiac baroreflex dysfunction in patients with pulmonary arterial hypertension.
Does the peripheral chemoreflex play a role in cardiac baroreflex dysfunction in patients with pulmonary arterial hypertension (PAH)?

Does the peripheral chemoreflex play a role in cardiac baroreflex dysfunction in patients with pulmonary arterial hypertension (PAH)?
In patients with nonhypoxemic PAH, cardiac baroreflex dysfunction is present and associated with poorer functional capacity, but is not mediated by the peripheral chemoreflex.

Baroreflex dysfunction associates with poorer capacity in nonhypoxemic PAH without chemoreflex mediation; leaves open alternative mechanisms and clinical relevance.
Does the peripheral chemoreflex play a role in cardiac baroreflex dysfunction in patients with pulmonary arterial hypertension (PAH)? Here we provide new evidence of cardiac baroreflex dysfunction under spontaneous and, most notably, provoked blood pressure fluctuations in patients with nonhypoxemic PAH. Importantly, impaired cardiac baroreflex effectiveness during provoked blood pressure fluctuations was independently associated with poorer functional capacity. Finally, our results indicated that the peripheral chemoreflex did not mediate cardiac baroreflex dysfunction among those patients.

Paula‐Ribeiro et al. (2021) conducted an observational in Pulmonary arterial hypertension. Peripheral chemoreflex was evaluated on Cardiac baroreflex dysfunction and functional capacity. Impaired cardiac baroreflex effectiveness during provoked blood pressure fluctuations was associated with poorer functional capacity in nonhypoxemic PAH, and was not mediated by the peripheral chemoreflex.
