Why the study?

Sleep disorders are very common in patients with heart disease, prompting an evaluation of how cardiac rehabilitation affects sleep quality, quality of life, anxiety, depression, and functional capacity.

Does an 8-week multidisciplinary cardiac rehabilitation program improve sleep quality in patients with heart disease?

Population

240 patients with heart disease in a cardiac rehabilitation program

Comparison

Pre-test vs post-test after an 8-week cardiac rehabilitation program

Design

Pre-test/post-test study

Follow-up

8 weeks

Key result

An 8-week multidisciplinary cardiac rehabilitation program significantly improved sleep quality in heart disease patients, reducing the median Pittsburgh Sleep Quality Index score from 7 to 6 (p=0.008).

Overview

Non-clinically significant sleep gains with cardiac rehab should not change practice; leaves open causality and optimal program effects.

Structured PICO

Does an 8-week multidisciplinary cardiac rehabilitation program improve sleep quality in patients with heart disease?

P
Population
240 clinically stable patients with heart disease (revascularization, heart failure, or valve surgery) who underwent an 8-week multidisciplinary cardiac rehabilitation program.
I
Intervention
Multidisciplinary Cardiac Rehabilitation Program (MCRP) for 8 weeks, consisting of programmed physical training (aerobic and strength), health education, and psychological counseling. Frequency was 2 sessions/week for low-risk patients and 3 sessions/week for medium and high-risk patients.
O
Outcome
Sleep quality measured by the Pittsburgh Sleep Quality Index (PSQI) at the end of the 8-week programpatient reported

Main Result

Effect estimate: -1.00 median change

Absolute Event Rate: 6% vs 7%

p-value: p=0.008

An 8-week multidisciplinary cardiac rehabilitation program statistically improved sleep quality, quality of life, anxiety, depression, and functional capacity in heart disease patients, though the change in sleep quality was not clinically significant.

Limitations

  • Single group pretest-posttest design without a control group.
  • The statistically significant improvement in sleep quality did not reach the predefined minimum clinically significant difference of 3 points.
  • Patients with severe heart failure (NYHA Class IV) were excluded, limiting generalizability to that subgroup.
  • Heterogeneity of the populations studied
  • Heterogeneity of chosen exercise modalities

Lodi-Rizzini et al. (2021) conducted an observational in Heart disease (revascularization, heart failure, or valve surgery) (n=240). Multidisciplinary Cardiac Rehabilitation Program vs. Baseline (pre-intervention) was evaluated on Pittsburgh Sleep Quality Index (PSQI) total score (-1.00 median change, p=0.008). An 8-week multidisciplinary cardiac rehabilitation program significantly improved sleep quality in heart disease patients, reducing the median Pittsburgh Sleep Quality Index score from 7 to 6 (p=0.008).
