Does selective ethanol ablation targeting the distal vein of Marshall terminate peri-left atrial appendage reentrant atrial tachycardia?
Selective ethanol ablation of the distal vein of Marshall can terminate peri-left atrial appendage reentrant atrial tachycardia and create conduction block along the mitral isthmus.
Some reentrant atrial tachycardias (ATs) after a percutaneous or surgical ablation of atrial fibrillation (AF) include the Marshall bundle (MB) in reentrant circuits, such as perimitral ATs, or ridge-related or MB-related reentrant ATs.1–11 An ethanol infusion into the vein of Marshall (VOM) can create an ablation lesion along the posterolateral left atrium (LA) and ridge between the LA appendage (LAA) and left pulmonary veins (PVs) and can be useful for terminating those MB-related ATs and creating conduction block along the mitral isthmus (MI) between the left PVs and posterolateral mitral annulus (MA).

Nakamura et al. (2022) conducted a case report in Reentrant atrial tachycardias after atrial fibrillation ablation. Selective ethanol ablation targeting the distal vein of Marshall was evaluated. Ethanol infusion into the vein of Marshall can create an ablation lesion useful for terminating Marshall bundle-related reentrant atrial tachycardias.
