Introduction and Objective: Youth-onset type 2 diabetes (Y-T2D) is characterized by early complications including cardiac autonomic neuropathy. Reduced heart rate variability (HRV) is an early marker of autonomic dysfunction. We compared HRV in controls with obesity versus those with Y-T2D to assess autonomic function. Methods: This interim analysis of a prospective study included 19 Y-T2D and 6 controls with obesity who wore Bittium Faros 180 monitors to assess heart rate (HR) and SDNN, a measure of overall HRV, over 24-h and for 1-minute intervals before, during, and after an Astrand-Rhyming submaximal exercise test. Groups were compared by Welch’s t-tests or Fisher’s exact tests, and HRV over time by a two-way ANOVA for repeated measures. Results: Baseline characteristics were similar by group (mean±SD: age 20±3 years, 52% female, 76% Black, resting HR 78±13 bpm, SBP 120±13 mmHg, BMI 36±6 kg/m², estimated VO2 max: 21.2±8.1 mL/kg/min). Despite no 24-h group differences in HRV (all P0.05), Y-T2D showed trends towards higher HR and lower SDNN. During exercise, significant group-by-time interactions were observed for HR and SDNN (both P0.05), with the greatest differences during recovery, when Y-T2D had persistently higher HR and lower SDNN. Conclusion: Despite similar 24-h HRV, Y-T2D demonstrated reduced post-exercise autonomic recovery, suggesting early signs of cardiac autonomic dysfunction. Disclosure I.N. Kacker: None. M.M.
