INTRODUCTION: The present study aimed to assess the relationship between the triglyceride glucose (TyG) index and early right ventricular dysfunction (RVD) in patients with chronic obstructive pulmonary disease (COPD). METHODS: The study included 100 patients with COPD and 50 age and sex-matched healthy controls. All patients were subjected to careful history taking and thorough general, chest, and cardiac examinations. Patients' symptoms were evaluated using the modified Medical Research Council (mMRC) dyspnea scale and the COPD Assessment Test (CAT). All images of the right heart were obtained using a GE Vivid 7 with a 3S transducer (1.5-3.6 MHz). Tricuspid annular plane systolic excursion (TAPSE) was calculated by measuring the distance of the RV annular segment's systolic excursion along its longitudinal plane from a conventional apical 4-chamber window. TAPSE < 17 indicated RVD. RESULTS: Patients were found to have a significantly higher TyG index compared to controls (4.86 ± 0.09 versus 4.54 ± 0.1, p <0.001). According to echocardiographic findings, there were 33 patients with RVD. Multivariate logistic regression analysis identified patients' age OR (95% CI): 1.66 (1.45-1.97), p =0.034, disease duration OR (95% CI): 2.07 (1.06-4.06), p =0.035, COPD severity OR (95% CI): 7.71 (1.23-48.12), p =0.041 and TyG index OR (95% CI): 4.13 (1.02-16.74), p =0.047 as predictors of RVD. CONCLUSION: In patients with COPD, early RVD is related to the TyG index. This finding emphasizes the importance of biometabolic factors in the pathogenesis and management of COPD.
