Key result
Hypertension is linked to ~96% higher odds of AVB requiring pacemaker therapy versus SSS.

Hypertension is a significant independent risk factor for atrioventricular block requiring pacemaker implantation in Japanese patients.
Hypertension was independently associated with AVB; leaves open whether BP control prevents conduction disease.
Background Risk factors and comorbidities for atrioventricular block (AVB) have not been well known in Japanese people. The aim of this study was to clarify the risk factors and comorbidities for AVB in recent years. Methods Yokohama cardiovascular implantable electronic devices (Y-CIEDs) Registry is a multicenter, prospective study of patients undergoing a newly implantation of CIEDs. A total of 872 consecutive patients with a newly implantation of pacemaker from 2020 to 2024 were analyzed. The enrolled patients were divided into two patients; AVB group (n=570), and sick sinus syndrome group (n=244), and clinical backgrounds were compared between two groups. Results The mean age was 78.8 years and 55.6% was male. The proportion of patients with hypertension in AVB group was significantly higher than those in SSS group (75.2% vs. 60.0%, p<0.001). In multivariate analysis controlling for age and sex, hypertension was an independent risk factor for AVB (OR, 1.959; p<0.001). Furthermore, within the AVB group, patients with chronic atrial fibrillation had a higher prevalence of hypertension, chronic kidney disease, and cerebral infarction compared to patients without chronic atrial fibrillation (87.1% vs. 74.6%, 45.2% vs. 22.6%, 13.0% vs. 6.3%, respectively). Conclusions In this recent multicenter study, hypertension was an independent risk factor for AVB requiring pacemaker therapy. Especially, patients with both AVB and chronic atrial fibrillation had multiple comorbidities.

Hosoda et al. (2026) conducted an observational in Atrioventricular block and sick sinus syndrome (n=872). Hypertension vs. Absence of hypertension was evaluated on Atrioventricular block requiring pacemaker therapy (OR 1.959, p=<0.001). Hypertension was an independent risk factor for atrioventricular block requiring pacemaker therapy compared to sick sinus syndrome (OR 1.959; p<0.001).
