Key result
Transapical TAVI was successfully performed in an 83-year-old female with severe aortic stenosis and chronic type A dissection, resulting in excellent clinical condition without cardiac symptoms at 30 months.
Why the study?
Advancements in technology and experience have enabled transcatheter aortic valve implantation use in challenging anatomies, such as severe aortic pathology.

Transapical TAVI can be a viable alternative for treating severe aortic stenosis in patients with complex aortic pathology such as chronic type A dissection.
TAVI may be feasible in post-repair chronic type A dissection with severe AS; leaves open need for systematic evaluation before broader use.
Summary Background Transcatheter aortic valve implantation (TAVI) has been a routine procedure for quite some years. Advancements in technology and experience have enabled its use in even more challenging anatomies. Case report An 83-year-old female patient with severe aortic pathology due to chronic type A dissection after supracoronary ascending aortic repair was suffering from progressive dyspnea and recurrent cardiac decompensations due to severe aortic stenosis. She was successfully treated with a transapical TAVI procedure by our interdisciplinary heart team. Conclusion Transapical TAVI is a good alternative for overcoming severe aortic pathology when treating aortic stenosis.

Mayer et al. (2022) conducted a case report in Severe aortic stenosis with chronic type A dissection (n=1). Transapical TAVI was evaluated on Procedural success and symptom relief. Transapical TAVI was successfully performed in an 83-year-old female with severe aortic stenosis and chronic type A dissection, resulting in excellent clinical condition without cardiac symptoms at 30 months.
