目的: 1992年, Brugadaらは特発性心室細動群の中に心電図上, 右脚ブロックパターンにST上昇 (RB-ST) を伴う一群のあることを報告した.この心電図波形が, 心室細動を予知する情報になる可能性があるとして, 現在注目されている.しかし, この心電図所見の疫学的情報はなく, 本報告が世界で初めての報告である.対象: 健康診断の目的で心電図を記録した小, 中学生 (9, 569人) と成人 (4, 092人) , および当科受診者 (8, 366人) の計22, 027人を対象に疫学調査を行なった.結果: (1) RB-ST型心電図は, 小, 中学生の0.01% (1人) , 成人の0.07% (3人) , 受診者の0.09% (8人) , 全体の口.05% (12人) であった. (2) RB-ST型はいずれも男性で, 年齢は14～71歳と層が広く, 明らかな基礎心疾患はなかった. (3) 失神の既往は12人中4人 (33%) で内3例は心室細動 (Vf) を認め, 1例は原因不明であり, Vfと診断し得た例は全例40歳以下であった.総括: RB-ST型心電図はきわめてまれであるが, 広い年齢層に認められ, また, 男性に多く, Vfとの関連は比較的若年者に見られた.
