Echocardiography-guided heart rate modulation using ivabradine, targeting zero overlap between diastolic filling inflow waves, is proposed to maximize cardiac output and facilitate reverse remodeling.
Why the study?
Heart rate modulation with ivabradine improves mortality and morbidity in systolic dysfunction, but the optimal target heart rate remains uncertain.
Does echocardiography-guided ivabradine therapy improve outcomes compared to using an absolute target heart rate in patients with systolic dysfunction?
Does echocardiography-guided ivabradine therapy improve outcomes compared to using an absolute target heart rate in patients with systolic dysfunction?
Echocardiography-guided ivabradine therapy is proposed as a novel strategy to optimize heart rate modulation in systolic dysfunction, requiring prospective validation.
Echocardiography-guided ivabradine may optimize output in systolic dysfunction; leaves open superiority to fixed-rate targets in randomized trials.
Heart rate modulation therapy using ivabradine improves mortality and morbidity in patients with systolic dysfunction. However, a target heart rate remains uncertain. Echocardiography-guided ivabradine therapy, in which we attempt to approach zero overlap between two diastolic filling inflow waves, has recently been proposed to maximize cardiac output, facilitate reverse remodeling, and reduce mortality and morbidity, instead of using an absolute value for the target heart rate. Prospective studies are needed to validate the clinical implication of these therapeutic strategies. Also, this concept should be expanded to other clinical scenarios.

Imamura et al. (2021) conducted a review in Heart failure with systolic dysfunction. Echocardiography-guided ivabradine therapy vs. Conventional treatment / Absolute target heart rate was evaluated. Echocardiography-guided heart rate modulation using ivabradine, targeting zero overlap between diastolic filling inflow waves, is proposed to maximize cardiac output and facilitate reverse remodeling.
