1 42-year-old female with idiopathic restrictive cardiomyopathy, persistent atrial fibrillation status post pulmonary vein isolation, premature ventricular complex ablation, systolic dysfunction, and incessant slow narrow complex tachycardia (AVNRT) with baseline first-degree AV block (PR 320 ms) and no anterograde fast pathway conduction.

Intervention
Radiofrequency ablation of the retrograde fast pathway in the anteroseptal region.

Termination of tachycardia and non-inducibility

In patients with typical AVNRT and prolonged PR interval or poor anterograde fast pathway conduction, fast pathway ablation is an effective alternative to slow pathway ablation.
