Implication

May signal heightened platelet reactivity in severe OSA without traditional risks; hypothesis-generating for prospective outcome studies.

Structured PICO

Is severe obstructive sleep apnea syndrome associated with increased mean platelet volume in patients without traditional cardiovascular risk factors?

P
Population
124 subjects aged 30-60 years, including 30 with mild obstructive sleep apnea syndrome (OSAS), 32 with moderate OSAS, 31 with severe OSAS, and 31 healthy controls. Excluded patients with impaired cardiopulmonary function, coronary artery disease, valvular disease, atrial fibrillation, congenital heart disease, hypertension, diabetes mellitus, dyslipidemia, chronic alcoholism, smoking, malignancy, thyroid disorders, prolonged NSAID/anticoagulant use, and renal/liver insufficiency.
C
Comparator
Healthy control subjects matched for age, sex, and body mass index
O
Outcome
Mean platelet volume (MPV)surrogate

Severe obstructive sleep apnea syndrome is associated with increased mean platelet volume, suggesting heightened platelet activation and atherothrombotic risk even in the absence of traditional cardiovascular risk factors.

Limitations

  • small sample size
  • use of Epworth's method and the Berlin scale rather than AHI in the selection of control individuals
  • unable to assess the effects of possible increased blood pressure during sleeping which could be induced by apnea and hypopnea episodes
