Why the study?
Ascending aortic thrombosis is rare and usually discovered following peripheral embolic symptoms, making asymptomatic cases extremely uncommon.
Surgical thrombectomy under hypothermic circulatory arrest can be safely performed for incidentally discovered asymptomatic ascending aortic thrombus to prevent embolic events.
May support surgical thrombectomy for asymptomatic ascending aortic thrombus; hypothesis-generating and leaves open need for comparative data.
症例は70歳男性.他院で食道癌化学療法中(5FU+CDDP)に腹痛の原因精査目的で造影CTを施行されたところ,偶然上行大動脈後面にlow-density massを認めた.血栓を疑いヘパリンの投与を開始するも腫瘤の縮小傾向はなく,摘出術目的に当院転院搬送となった.腹痛は自然軽快し,その他明らかな塞栓症状は認めなかった.手術は低体温循環停止下に血栓摘除術,大動脈壁修復術を施行した.術中の経大動脈エコーでは,血栓が血流によって浮動する様子が観察できた.術後経過は良好で第18病日に退院となった.上行大動脈内血栓症は稀な疾患であり,その多くは末梢動脈の塞栓症状で発見されるが,無症候で発見される症例はきわめて少ない.今回,塞栓症状なく偶然発見された上行大動脈内血栓症に対して,血栓除去術を施行し良好な結果を得たので報告する.
