Overview

May aid fetal counseling in double discordance; leaves open need for larger prospective validation before practice change.

Structured PICO

P
Population
34 consecutive fetuses with a prenatal diagnosis of atrioventricular and ventriculoarterial discordance (double discordance) seen between January 1993 and December 2003 at a tertiary centre for fetal cardiology.
I
Intervention
Fetal echocardiography (retrospective analysis of videotape recordings)
O
Outcome
Echocardiographic features including reversed differential insertion, identification of the right ventricle through the moderator band, and the origin and orientation of the great arteries, as well as associated cardiac abnormalities and outcome.

Fetal echocardiography can reliably identify double discordance through specific features like reversed differential insertion, and while isolated cases are rare, short-term survival for continued pregnancies is favorable (82%).
