Overview

High peripartum hemorrhage and postpartum deaths call for caution with anticoagulants in pregnancy; leaves open optimal regimens for mechanical valves.

Structured PICO

What are the adverse effects and safety profile of different anticoagulant therapy strategies during pregnancy?

P
Population
50 women (54 pregnancies) treated with anticoagulants, most frequently for mechanical heart valve prosthesis (68%).
I
Intervention
Anticoagulant therapy during pregnancy. Group I (n=43) received heparin substitution for oral anticoagulants from the 6th to 12th week of gestation. All patients received vitamin K antagonists during the 2nd and 3rd trimesters, and heparin after 36 weeks until 10 days postpartum.
C
Comparator
Group II (implied n=11) where oral anticoagulants (acenocoumarol) were not substituted with heparin during the first trimester.
O
Outcome
Adverse effects of anticoagulant therapy (including thrombosis, spontaneous abortion, cardiac complications, hemorrhagic complications, maternal death, and embryopathy).safety

Anticoagulant therapy during pregnancy, particularly for mechanical heart valves, carries a significant risk of peripartum hemorrhagic complications and maternal mortality.
