Overview

Does not support clinical action in small-infarct patients with preserved LVEF; leaves open prognostic value in higher-risk AMI.

Structured PICO

P
Population
67 consecutive patients with acute myocardial infarction (AMI) treated with early reperfusion therapy, mean LVEF 58+/-15%
I
Intervention
Radionuclide imaging of myocardial resting perfusion using (201)thallium and presynaptic sympathetic innervation using (123)I-metaiodobenzylguanidine (MIBG), combined with electrophysiologic evaluation (resting ECG, signal-averaged ECG, 24-h Holter)
O
Outcome
Correlation between perfusion/innervation mismatch and electrophysiologic parameters (repolarisation and depolarisation)surrogate

In patients with early reperfusion for AMI, viable but denervated myocardium is common and correlates with slow depolarisation and repolarisation, though it appears to have little influence on clinical outcomes in those with small infarcts and preserved LVEF.
