A 78-year-old man presented with progressive difficulty walking, and changes in mental status. However, his brain magnetic resonance imaging was unremarkable. The possibility of paraneoplastic syndrome was raised, and an FDG-PET/CT of the body was performed to search for potential malignancy. Interestingly, there was diffusely increased FDG uptake along the lower spinal cord and sciatic nerves, which was subsequently confirmed to be due to neuropathy following nerve biopsy.
