Implication

High early mortality after reoperative AVR warrants caution in patient selection; observational data leaves open whether contemporary strategies can improve survival.

Structured PICO

P
Population
104 patients undergoing aortic valve replacement (AVR) after previous coronary artery bypass graft (CABG) surgery, mean age 67 years, 83% male. 70% had congestive heart failure, 96% had multivessel coronary artery disease. Diagnosis was aortic stenosis in 68%, aortic insufficiency in 16%, and combined in 16%.

Intervention

O
Outcome
Early mortality and late mortalityhard clinical

Aortic valve replacement after previous CABG carries a high early mortality (14%), but long-term survival for those who survive the early postoperative period is favorable.
