Retrospective analysis of 75 patients with elevated parathyroid hormone levels and suspected primary hyperparathyroidism between September 2023 and November 2024.
All patients underwent 99mTc-MIBI/99mTcO4− dual-isotope SPECT/CT fusion imaging and neck ultrasonography, using post-surgical histopathology as the reference standard.
On a per-lesion basis across 74 confirmed parathyroid lesions in 68 patients, dual-isotope SPECT/CT demonstrated significantly higher sensitivity (97.01% vs. 79.10%, P < 0.001) and positive predictive value (85.87% vs. 80.72%, P = 0.006) than ultrasonography.
Per-patient diagnostic sensitivity was 94.12% (64/68) for SPECT/CT compared to 86.76% (59/68) for ultrasonography.
In classical primary hyperparathyroidism, SPECT/CT retained superior sensitivity (96.22% vs. 84.91%, P < 0.001) and positive predictive value (94.64% vs. 82.45%, P = 0.039), and adenoma volume correlated moderately with corrected serum calcium (r = 0.435, P = 0.003) and parathyroid hormone levels (r = 0.303, P = 0.045).
