老年者における pseudohypertension の頻度を調べる目的で, 65歳以上の老年者59例について, マンシェット法による血圧値と上腕動脈内にカテーテルを挿入することにより得られた直接法の値とを比較検討した. 収縮期血圧に関しては間接法では161.5±26.7mmHg (mean±SD), 直接法では169.2±26.7mmHgで前者は後者に比して有意に低値であり (p<0.05), 間接法の値が直接法の値を10mmHg以上上回るいわゆる pseudohypertension は1例も認められなかった. 拡張期血圧値は両測定法間にr=0.86の高い相関を認めたが両測定法による血圧値の有意差は認められなかった. Osler 法陽性者と陰性者との間には収縮期血圧, 拡張期血圧とも両測定法による血圧値の有意差が認められなかった.脈波速度と収縮期血圧との間には有意の相関が認められた (Y=11.4X±66.1, r=0.65). しかし脈波速度と両測定法による圧差との間には収縮期血圧, 拡張期血圧とも, 相関が認められなかった.今回の我々の成績からは老年者においても pseudohypertension の頻度は非常に低く, 観血的な直接法による血圧測定の必要は少ないことが示された.
