巨大左房(GLA)を伴った僧帽弁閉鎖不全(MR),三尖弁閉鎖不全(TR)症例に,僧帽弁形成(MVP)および三尖弁輪縫縮(TAP)にあわせてspiral plication methodによる左房縫縮術(LAP)を行い,良好な結果を得たので若干の文献的考察を加え報告する.症例は66歳,男性で,胸部X線写真では,心胸郭比(CTR)92%,右側CTR88.4%,心エコー上,左房径(LAD)は107mmで,IV度のMRとII度のTRを認めた.手術ではsuperior transseptal approachの切開線より,左肺静脈の外側から左心耳を切除しつつ僧帽弁輪をまわり,右側左房にいたる連続した線を描きながら左房壁を切除,縫縮した結果左房の螺旋形縫縮(spiral plication)となった.術後32日目の胸部X線写真ではCTR71%,右側CTR54.5%と縮小,心エコー上もLADは67mmと短縮し,このspiral LAPは,有効な縫縮術と考えられた.
