Key result
Compared to the Orsiro stent, the Combo stent was associated with a higher risk of target lesion failure at 1 year in patients with ACS (6.7% vs 4.1%; IRR 1.65) and without ACS (6.0% vs 3.2%).
Why the study?
Target lesion failure is higher after modern drug-eluting stents in ACS, but whether the SORT OUT X trial results apply to patients with and without ACS remains unknown.
Does the dual-therapy CD34 antibody-covered sirolimus-eluting Combo stent reduce target lesion failure compared to the sirolimus-eluting Orsiro stent in patients with and without acute coronary syndrome?

Does the dual-therapy CD34 antibody-covered sirolimus-eluting Combo stent reduce target lesion failure compared to the sirolimus-eluting Orsiro stent in patients with and without acute coronary syndrome?
The dual-therapy CD34 antibody-covered sirolimus-eluting Combo stent is associated with a higher risk of target lesion failure at 1 year compared to the sirolimus-eluting Orsiro stent, regardless of ACS presentation, driven mainly by higher rates of target lesion revascularization.
Combo stent increases target lesion failure risk versus Orsiro regardless of ACS; challenges dual-therapy stent superiority.

Jakobsen et al. (2022) conducted an RCT in Acute coronary syndrome (ACS) and non-ACS (n=3,146). Dual-therapy CD34 antibody-covered sirolimus-eluting Combo stent (DTS) vs. Sirolimus-eluting Orsiro stent (O-SES) was evaluated on Target lesion failure (TLF), a composite of cardiac death, target-lesion myocardial infarction (MI), or target lesion revascularization (TLR) within 1 year (IRR 1.65, 95% CI 1.08-2.52). Compared to the Orsiro stent, the Combo stent was associated with a higher risk of target lesion failure at 1 year in patients with ACS (6.7% vs 4.1%; IRR 1.65) and without ACS (6.0% vs 3.2%).
