Key result
Pediatric HF case features incessant wide QRS tachycardia interrupted by narrow complexes in 4-complex cycles.
Why the study?
The mechanism underlying wide QRS complex tachycardia with group beating in a young patient with heart failure was unclear.
Case Report (n=1)
A case report presenting an 11-year-old girl with heart failure and a complex wide QRS tachycardia to discuss its underlying mechanism.
A healthy 11-year-old girl was referred to our center because of dyspnea and palpitations for several weeks. A transthoracic echocardiography demonstrated a left ventricular ejection fraction of 35%. Electrocardiograms were taken and showed incessant wide QRS complex tachycardia regularly interrupted by a narrow QRS complex. A closer inspection revealed repeating similar cycles, each consisting of 4 complexes. What is the mechanism of this phenomenon?

Waroux et al. (2012) conducted a case report in Heart failure and wide QRS complex tachycardia (n=1). An 11-year-old girl with heart failure presented with an incessant wide QRS complex tachycardia regularly interrupted by a narrow QRS complex in repeating 4-complex cycles.
