Epinastine does not alter ECG parameters in rats while terfenadine induces QT and PR prolongation.
Why the study?
The electrocardiographic effects of epinastine and terfenadine in rats were investigated from a pharmacokinetic and pharmacodynamic perspective to understand their impact on ECG parameters.
Epinastine may have a safety advantage over terfenadine by avoiding adverse electrocardiographic reactions such as QT prolongation.

Suggests epinastine may avoid terfenadine cardiotoxicity; hypothesis-generating in rats and requires human confirmation before any clinical consideration.
Ohtani et al. (1997) studied this question. Epinastine hydrochloride vs. Terfenadine (5, 10 or 15 mg kg-1 h-1) was evaluated on Changes in QT interval, heart rate, and PR interval. Epinastine did not affect ECG parameters at high plasma concentrations, whereas terfenadine induced QT and PR prolongation and bradycardia in an infusion-rate-dependent manner in rats.
