Key result
Frusemide/amiloride matches hydrochlorothiazide/amiloride for BP reduction with less potassium and sodium loss.
Why the study?
The comparative efficacy and tolerability of frusemide plus amiloride versus hydrochlorothiazide plus amiloride as first-line treatment in mild to moderate essential hypertension was not established.
Does furosemide plus amiloride improve blood pressure control and tolerability compared to hydrochlorothiazide plus amiloride in patients with mild to moderate essential hypertension?
Comparison
Frusemide 40 mg plus amiloride 5 mg vs hydrochlorothiazide 50 mg plus amiloride 5 mg

Does furosemide plus amiloride improve blood pressure control and tolerability compared to hydrochlorothiazide plus amiloride in patients with mild to moderate essential hypertension?
Furosemide plus amiloride is as effective as hydrochlorothiazide plus amiloride for blood pressure reduction but avoids significant reductions in plasma potassium and sodium levels.
Backhouse et al. (1988) conducted an RCT in mild to moderate essential hypertension (n=44). Frusemide plus amiloride vs. Hydrochlorothiazide plus amiloride (50 mg / 5 mg per tablet) was evaluated on Blood pressure reduction. Frusemide/amiloride and hydrochlorothiazide/amiloride both effectively reduced blood pressure, but hydrochlorothiazide/amiloride significantly reduced plasma potassium and sodium levels.
