Poor glycemic control and longer diabetes duration are linked to worsening LV strain and filling pressures.
Why the study?
Classic echocardiographic methods are not sensitive to detect subclinical early LV systolic dysfunction in diabetic heart disease, necessitating identification of determinants of diabetes' deleterious myocardial effects.
Does poor glycemic control and longer duration of diabetes worsen subclinical left ventricular systolic function and filling pressures in diabetic patients?

Does poor glycemic control and longer duration of diabetes worsen subclinical left ventricular systolic function and filling pressures in diabetic patients?
Poor glycemic control and longer duration of diabetes are associated with subclinical left ventricular dysfunction as detected by reduced global longitudinal strain.
Speckle tracking may detect subclinical LV dysfunction in diabetes; leaves open independent roles of duration and control.
Background and Aims: Early discovery of diabetic heart disease is a dignified mission. Classic echocardiographic method is not sensitive to detect subclinical early LV systolic dysfunction. Early deleterious effects of DM on LV systolic function appeared longitudinally by speckle tracking. We aimed to uncover the determinants of deleterious effects of DM on the myocardium using echocardiographic indices, considering the duration of DM as well as the state of DM control. Methods and Results: 52 diabetic patients were enrolled in two groups (Group I; uncontrolled DM with HbA1c ≥ 6.5%) and group II; controlled DM with HbA1c5 years or 5 years (-15.7i‚±2.8%). (t=8.9, p=0.05). Conclusion: The duration of DM is strongly correlated with reduction of GLS and elevation of LV filling pressure. Poor glycemic control (HbA1c>6.5%), leads to reduction in LV GLS, which is associated with preclinical LV dysfunction and elevated LV filling pressure.

A 2016 study conducted an observational in Diabetes Mellitus (n=52). Poor glycemic control (HbA1c ≥ 6.5%) and longer diabetes duration vs. Controlled diabetes was evaluated on Reduction of global longitudinal strain (GLS) and elevation of LV filling pressure (p=0.05). Poor glycemic control (HbA1c > 6.5%) and longer diabetes duration were associated with reduced left ventricular global longitudinal strain and elevated filling pressure (p=0.05).
