Rheocarna combined with sacubitril/valsartan triggers severe hypotension, dropping systolic BP by ~80 mmHg.
Why the study?
The safety and interaction risk of combining sacubitril/valsartan with Rheocarna therapy, particularly regarding shock events, is not well established.
Case Report (n=1)
No
The combination of Rheocarna and sacubitril/valsartan can cause severe shock similar to ACE inhibitors due to bradykinin accumulation, necessitating careful monitoring or temporary discontinuation of the drug.
Sacubitril/valsartan with Rheocarna may precipitate shock; single case leaves open need for label revision or routine precautions.

Gen et al. (2026) conducted a case report in Refractory foot ulcer in a hemodialysis patient (n=1). Rheocarna therapy during sacubitril/valsartan treatment was evaluated on Severe hypotension (shock). Concomitant use of Rheocarna therapy and sacubitril/valsartan triggered severe hypotension, with blood pressure dropping from 149/79 mmHg to 70/40 mmHg within 10 minutes of initiation.
