Excision of subvalvular remnants resolves severe intermittent transvalvular MR and heart failure symptoms post-MVR.
Why the study?
Complications of mitral valve replacement with subvalvular preservation, including rare intermittent severe transvalvular mitral regurgitation, require further characterization.
Case Report (n=1)
Alerts clinicians to subvalvular remnants as reversible cause of intermittent prosthetic MR; leaves open optimal preservation strategies during MVR.
Mitral valve replacement with subvalvular preservation is a favorable technique to protect left ventricular function and improve long-term survival. However, complications of the procedure should be considered. We report the case of a patient with a history of prosthetic mitral valve replacement with severe intermittent transvalvular mitral regurgitation and reduced ejection fraction. (Level of Difficulty: Advanced.)

Tan et al. (2021) conducted a case report in Severe intermittent transvalvular mitral valve regurgitation (n=1). Resection of subvalvular apparatus remnants was evaluated on Valve function and symptom resolution. Surgical excision of subvalvular apparatus remnants successfully treated severe intermittent transvalvular mitral regurgitation and resolved heart failure symptoms in a patient with a prior mitral valve replacement.
