Key result
Only ~18% of high-risk secondary prevention patients achieve guideline LDL-C targets in Japanese primary care.
Why the study?
The Japan Atherosclerosis Society revised LDL-C targets and patient criteria in 2022, necessitating assessment of LDL-C target achievement in Japanese dyslipidemia patients.
Does real-world lipid-lowering therapy achieve guideline-directed LDL-C targets in Japanese patients with dyslipidemia?
Observational (n=174,814)
Yes
Does real-world lipid-lowering therapy achieve guideline-directed LDL-C targets in Japanese patients with dyslipidemia?
A large proportion of high-risk Japanese patients with dyslipidemia fail to achieve the revised 2022 JAS LDL-C targets, highlighting a significant gap in real-world lipid-lowering therapy optimization.
Low achievement rates signal implementation gaps in high-risk groups; leaves open whether targeted interventions improve 2022 JAS guideline adherence in Japanese primary care.
Background: Atherosclerotic cardiovascular disease (ASCVD) remains a major cause of mortality in Japan. The Japan Atherosclerosis Society (JAS) published revised guidelines in 2022 (JASGL2022), introducing updated low-density lipoprotein cholesterol (LDL-C) targets and patient criteria. Therefore, in this study, we assessed LDL-C target achievement among Japanese patients with dyslipidemia, as defined in the JASGL2022.

Ishii et al. (2026) conducted an observational in Dyslipidemia (n=174,814). Lipid-lowering therapy and care was evaluated on LDL-C target achievement rate. In a nationwide Japanese primary care database, LDL-C target achievement rates based on the 2022 Japan Atherosclerosis Society guidelines were 18.1% for secondary prevention high-risk patients, 53.0% for secondary prevention, and 48.9% for primary prevention high-risk patients.
