The development of information technology in the healthcare sector encourages healthcare facilities to undergo digital transformation to improve service quality. Independent Midwifery Practice (PMB) X still relies on manual processes for patient registration, patient data recording, medical examinations, medication management, payments, and reporting. These processes may lead to service delays, recording errors, and difficulties in managing patient data. This study aims to design and implement a web-based patient service information system to support more effective and efficient service management. This study employed a qualitative method, with data collected through observation and interviews. The system was developed using the Waterfall model, which consists of requirements analysis, system design, implementation, testing, and maintenance. The system was developed using PHP and JavaScript, with MySQL as the database management system. The resulting web-based information system provides features for login, patient registration and data management, medical examinations, medical records, medication and prescription management, payments, receipt printing, and report generation. System design was supported by a Flowmap, Data Flow Diagram (DFD), and Entity Relationship Diagram (ERD). Testing using the black-box method showed that all tested scenarios passed. The system is expected to improve service effectiveness and facilitate computerized patient data management.
