Premarital health examination services are a form of preventive health care intended to identify couples’ health conditions before marriage. This study aims to design a web-based information system for premarital medical check-up registration that facilitates registration for prospective brides and grooms and assists public health center personnel in managing health service data. The system development method used was the Waterfall model, consisting of requirements analysis, system design, implementation, testing, and maintenance. Data collection techniques included observation, interviews, and literature review. The study produced a web-based information system design providing features for prospective bride and groom registration, user data management, uploading required documents, provision of information on examination stages, and report generation. The premarital medical check-up registration process at Cikalapa Public Health Center, Subang Regency, still faces several constraints, including non-integrated data recording, manual document management, and limited information regarding health examination procedures. These conditions may make administrative and service processes less effective and potentially increase waiting time for prospective brides and grooms. The proposed system is expected to improve the effectiveness and efficiency of premarital medical check-up administrative services at Cikalapa Public Health Center.
