The process of issuing death certificates at the Medical Records Unit of Kebonjati Hospital in Bandung is still carried out conventionally, resulting in delayed updates to patient status, an increased risk of recording errors and data duplication, and difficulties in verifying document authenticity. This study aims to design and develop a web-based Death Certificate Information System that uses QR codes for document validation to improve the management of deceased patients’ medical records. The system was developed using the Waterfall method, covering requirements analysis, system design, implementation, testing, and maintenance. The design was documented with a Context Diagram, Data Flow Diagram (DFD), Entity Relationship Diagram (ERD), and flowmap. The application was implemented with Python Flask as the backend framework, SQLite as the database, and HTML, HTMX, and Tailwind CSS for the user interface. Black box testing showed that all tested functions operated in accordance with user requirements without any functional errors. The system can manage data on deceased patients, generate death certificates equipped with QR codes for validation, and simplify the verification of document authenticity. The proposed system therefore improves the effectiveness, efficiency, and security of death certificate management at Kebonjati Hospital in Bandung.
