General patient registration services at Tambelang Community Health Center require integrated data management. This study aims to design, implement, and test a mobile-based general patient registration information system prototype using the Flutter framework, with data storage designed for cloud computing services. A descriptive qualitative approach was used to identify requirements through observation, interviews, documentation, and literature review. System development employed the Waterfall model through six stages: analysis, design, implementation, testing, deployment, and maintenance; operational deployment and maintenance were positioned as subsequent development activities. The study produced a prototype for managing patient, physician, clinic, medication, registration, examination-history, and service-recapitulation data. System modeling included a flowmap, context diagram, Entity Relationship Diagram (ERD), and Data Flow Diagram (DFD), with access rights designed for five roles: Administrator, Patient, Physician, Pharmacy Staff, and Head of the Community Health Center. Black-box testing of three scenarios—registration, storage of medical records and diagnoses together with prescriptions, and presentation of service recapitulation—resulted in a valid status for each scenario. These findings indicate functional conformity within the scenarios tested. Evaluation of service-time efficiency, user acceptance, security, and implementation of all access rights remains necessary in subsequent testing.
