Triple balloon inflations at rated burst pressure during PBED significantly reduced the side branch jailing ratio compared to a single inflation at nominal pressure (18.9% vs. 28.9%, P<0.05).
Why the study?
Although proximal balloon edge dilation (PBED) reduces side branch jailing in coronary bifurcation lesions, the jailing ratio remains high when a stent link is present on the carina after main branch stenting (link-connect type).
Does varying balloon pressure (nominal vs. rated burst) and inflation frequency (single vs. triple inflations) during the proximal balloon edge dilation (PBED) technique improve side branch jailing, stent apposition, and stent morphology in a bench model of link-connect type coronary bifurcation stenting?

Hypothesis-generating for PBED optimization in bifurcation stenting; leaves open translation to clinical outcomes.
Background: Coronary bifurcation lesions pose procedural challenges due to the need to maintain side branch (SB) patency. The proximal balloon edge dilation (PBED) technique has been introduced to reduce SB jailing by enhancing stent expansion at the SB ostium. However, even when using the PBED technique, the jailing ratio at the SB remains high in the presence of a stent link on the carina immediately after main branch stent implantation (link-connect type). This bench study aimed to optimize the PBED technique in link-connect type by investigating the effects of balloon pressure (nominal vs. rated burst) and inflation frequency (single vs. triple inflations) on SB jailing, stent apposition, and stent morphology.

Kume et al. (2026) studied Coronary bifurcation lesions (in vitro bench model) (n=10). Proximal balloon edge dilation (PBED) with triple inflations at rated burst pressure vs. PBED with single inflation at nominal pressure (12 atm for 10 seconds) was evaluated on Side branch (SB) jailing ratio (%) (p=<0.05). Triple balloon inflations at rated burst pressure during PBED significantly reduced the side branch jailing ratio compared to a single inflation at nominal pressure (18.9% vs. 28.9%, P<0.05).
