Introduction: Transseptal puncture (TSP) is a key step in left-sided interventional cardiac procedures. Radiofrequency (RF) energy for purpose-built TSP devices has been shown to improve safety, efficacy, and procedural efficiency. Current powered-wire options are not compatible with most transseptal sheaths and have been shown to rarely create inadvertent trauma when the active tip contacts the left atrium (LA) after crossing. The study describes an initial experience with the novel Hotwire™ system, which includes an RF generator with sensing algorithms and an RF-capable guidewire designed for broad sheath compatibility.
