Why the study?

Severe tricuspid regurgitation carries a poor prognosis without treatment, and transcatheter options are needed for patients who are not candidates for surgery.

Overview

Should not yet change practice for severe TR; leaves open the need for randomized trials to define its role.

Structured PICO

P
Population
Patients with severe symptomatic tricuspid regurgitation who are at high or prohibitive surgical risk and are not candidates for transcatheter tricuspid valve repair.
I
Intervention
Transcatheter tricuspid valve replacement (TTVR)

Transcatheter tricuspid valve replacement is an emerging and feasible therapeutic option for patients with severe tricuspid regurgitation at high surgical risk, requiring careful patient selection and multimodality imaging.

Limitations

  • Patients with severe right ventricular or left ventricular dysfunction are excluded from ongoing trials
  • Extreme dilatation of tricuspid annuli may not be amenable to TTVR due to limited availability of prosthetic valve sizes and the need for a larger sheath
  • The need for long-term anticoagulation adds challenges and may exclude patients at high risk of bleeding
  • Long-term data regarding prosthetic valve durability and clinical outcomes are currently lacking
  • Patients with severe RV or left ventricular dysfunction are excluded from ongoing trials
  • Patients with extreme dilatation of tricuspid annuli may not be amenable to TTVR due to the limited availability of prosthetic valve sizes and the need for a larger sheath
  • The need for anticoagulation adds yet another challenge and may exclude patients at high risk of bleeding
