Implication

Supports endothelial dysfunction as ischemia mechanism in hypertensive LVH; hypothesis-generating and should not yet change practice.

Structured PICO

Does hypertension with left ventricular hypertrophy reduce endothelium-dependent coronary vasodilation in human subjects?

P
Population
19 subjects total: 10 patients with left ventricular hypertrophy secondary to essential hypertension (HTN) and normal left ventricular systolic function, and 9 normal control subjects with no history of hypertension.
I
Intervention
Intracoronary infusion of the endothelium-dependent vasodilator acetylcholine (10(-8)-10(-6) M) and the endothelium-independent vasodilator adenosine (10(-6)-10(-4) M) into the left anterior descending coronary artery.
C
Comparator
Normal control subjects with no history of hypertension receiving the same intracoronary infusions.
O
Outcome
Changes in coronary blood flow (CBF) assessed using an intracoronary Doppler catheter and quantitative angiography.surrogate

Endothelium-dependent vasodilation is markedly impaired in the coronary microvessels of patients with hypertension and left ventricular hypertrophy, providing a potential mechanism for ischemic symptoms in the absence of obstructive coronary disease.
