Key result
Cardiac tissue maintains local Ang II production despite systemic suppression by captopril.
Why the study?
The origin of cardiac angiotensin I and II production and the effects of ACE inhibition on cardiac versus circulating angiotensin levels were unclear.
Population
Pigs weighing 25 to 30 kg
Comparison
Infusion of 125I-labeled Ang I or II with and without captopril treatment
Design
Preclinical infusion study with ACE inhibitor intervention

Suggests incomplete cardiac RAS suppression by ACEIs; hypothesis-generating for tissue-targeted therapies in HF.
Cardiac tissue can maintain local Angiotensin II production independently of circulating levels, even during ACE inhibitor therapy.
Kats et al. (1998) studied this question. 125I-labeled Ang I or II infusion and captopril vs. Untreated baseline was evaluated on Cardiac tissue and blood plasma concentrations of 125I-Ang I and II and endogenous Ang I and II. Most cardiac Ang II is produced at tissue sites by conversion of in situ-synthesized Ang I, and the heart can maintain Ang II production even when circulating Ang II is suppressed by captopril.
