A new conversion formula incorporating eGFR and BMI accurately estimated NT-proBNP from BNP, demonstrating a correlation coefficient of 0.942 in the internal validation set.
Why the study?
BNP and NT-proBNP are commonly used markers of heart failure, but a simple conversion formula between them had not yet been developed for clinical use.

A new conversion formula incorporating eGFR and BMI accurately converts BNP to NT-proBNP, which is particularly useful for estimating endogenous BNP levels in patients treated with sacubitril/valsartan.
May support BNP-to-NT-proBNP conversion in sacubitril/valsartan patients; leaves open need for prospective validation before routine use.
BACKGROUND: Although B-type natriuretic peptide (BNP) and N-terminal (NT)-proBNP are commonly used markers of heart failure, a simple conversion formula between these peptides has not yet been developed for clinical use. METHODS AND RESULTS: =0.900, P<0.0001). The correlation coefficients between the actual and converted values of log NT-proBNP in the internal and external validation sets were 0.942 (P<0.0001) and 0.891 (P<0.0001), respectively. We applied this formula to samples obtained from patients administered with sacubitril/valsartan. After treatment initiation, NT-proBNP levels decreased and actual BNP levels increased. However, the calculated BNP levels decreased roughly parallel to the NT-proBNP levels. CONCLUSIONS: This new and simple conversion formula of BNP and NT-proBNP with eGFR and BMI is potentially useful in clinical practice.

Ishihara et al. (2022) conducted an observational in Heart failure and cardiovascular disease (n=9,394). Conversion formula using eGFR and BMI vs. Actual measured NT-proBNP and BNP values was evaluated on Correlation between actual and converted values of log NT-proBNP in the internal validation set (r=0.942, p=<0.0001). A new conversion formula incorporating eGFR and BMI accurately estimated NT-proBNP from BNP, demonstrating a correlation coefficient of 0.942 in the internal validation set.
